2011 Summer Forms

All families must return the following forms each school year to the school office.

Please return the "2011 Summer Forms" by August 22, 2011.

Child Health Record Form is required for:

All students entering Kindergarten.
All new students entering Grades 1 through 8. Please note that any student transferring from out-of-state or from high risk areas of New Jersey, must have a Mantoux Turberculin test.
All students entering sixth grade in September 2011, the following two vaccines are required: Meningococcal vaccine (one dose) and Tdap vaccine booster (one dose).

Annual Athletic Pre-Participation Physical Examination Form is required for:

All students who will be admitted to school teams, such as basketball, softball or baseball.

Academic Fee 2011 -2012

After School Care Program

Honeywell

All new families and current families not registered with Honeywell must register with our Honeywell Instant Alert System at https://instantalert.honeywell.com.

For all Honeywell registered families, please remember to update your information i.e. new phone numbers, and also check and update your “Alert Setup”.

Picture Refusal Form

It will be our school policy to use children’s photos in publications, newsletters, newspapers, websites, etc. when the situation warrants. No child will be identified by name in any form of publication.

However, you have the option of not allowing your child’s image to be used. Please print and fill out the refusal form. (Refusal_of_Student_Picture_Form.pdf) A failure to return the form will indicate that you are allowing the school to use your child’s image. Please return the form to the school office by August 22, 2011.
